Died peacefully on January 5, 2013 with family at her side. Preceded in death by her husband Roy O. Jr. and son Allan. Survived by daughter-in-law June Dalhstrom; son LeRoy (Kathy) Dahlstrom; granddaughters Paula (Todd) Kuchenmeister, Jennifer (Matt) Nordahl, Suzanne (Mike) Slavik; grandson, Gregory (Karen) Dahlstrom and seven great grandchildren. Private interment Fort Snelling Cemetery. Hazel was a volunteer for the American Legion Auxiliary for 50 years and was a lifetime member. The family would like to express their gratitude and appreciation for the care that Hazel was given by HealthEast St. John's Hospital and Hospice (P2), including Dr. Carol Entwistle and the team of angelic RNs and CNAs. In lieu of flowers, the family requests that memorials be made to the American Cancer Society.
